What is ISS CareTrack?

ISS CareTrack is a developing documentation and workflow platform focused on the operational needs of Texas Individualized Skills and Socialization providers. It is intended to help providers move from fragmented paper-based documentation to a more structured digital workflow.

The system is being designed around Form 8615-style service documentation, role-based review, correction tracking, incident note workflows, billing packet preparation, reimbursement readiness, reports, and audit-friendly activity tracking.

Why it was created

ISS providers often need to coordinate documentation across staff, reviewers, administrators, and billing teams. When records are paper-based or scattered across disconnected tools, it becomes harder to know what has been completed, what still needs review, what was returned for correction, and which records are ready for billing follow-up.

ISS CareTrack was created to support a more transparent workflow where documentation status, reviewer decisions, correction notes, and billing readiness can be tracked more consistently.

Current platform focus

The current development focus is on building and stabilizing the core operational workflows that providers need before production deployment.

Documentation workflow Form 8615-style documentation, staff submissions, review status, correction requests, and approved-record tracking.
Review and correction workflow Reviewer approval, rejection, closure, return-for-correction decisions, reviewer notes, and attestation labels.
Incident note workflow Incident note creation, submission, review, correction, approval, rejection, closure, and print/readiness behavior.
Billing readiness Billing packet tracking, reimbursement readiness, invoice/payment logic, adjustment awareness, and reporting support.

Development and hosting direction

ISS CareTrack is currently being developed and tested with demo data only. The public website is separated from the application portal so the application can later be migrated to a HIPAA-conscious hosting environment while the public landing page may remain separately hosted.

Prototype notice ISS CareTrack is currently in prototype development. Real patient information, protected health information, official provider documentation, incident details, billing records, and Medicaid identifiers should not be entered until production hosting, security controls, policies, agreements, and compliance requirements are finalized.

Our practical goal

The goal of ISS CareTrack is to provide a focused, lightweight, and workflow-oriented platform that helps bridge the gap between clinical documentation needs, operational oversight, billing readiness, and information technology.

Rather than replacing every system a provider may already use, ISS CareTrack is being built to support a specific pain point: organizing ISS documentation from staff entry through review, correction, approval, billing preparation, and audit-friendly reporting.
